Cmix, which is a variant on Csound and part of the Music 5 family of audio 
programming languages/environments, will allow you to pass Perl code to, or 
allow you to work directly in Perl.  http://music.columbia.edu/cmc/RTcmix/

Also, Sony's Acid is a popular GUI software that combines MIDI and audio 
quite well if your not sure about the really steep learning curve of 
Csound/Cmix.
