An update: I think I understand, at least partly, what is going wrong. The Account list shows one entry for Costco Visa Card. The Account sidebar also shows one entry for Costco Visa Card. But whenever I do an OSU, the dialog box shows *two* entries for it. One of them always has its box unchecked and always asks for the password. I left the box unchecked, didn't supply the password, clicked "Apply" and then Update Now." The OSU then ran without an error. I then ran OSU again. This time the first thing that happened was that I got the message "Accounts at Citi Card for Use ID XXXXXX are not enabled correctly and need to be deactivated and reactivated to allow Quicken to properly get new transactions." and the OSU again completed without an error. So I assume that extra entry is causing the problem. It's not a giant problem if the only thing that happens is that the extra entry doesn't get updated and the real entry does, but I'd still like to get rid of the extra entry. Any suggestions on how to do that? I can't do it from the Account list, because it's not shown there.
