First of all, there is no right answer.  And the opinions you receive, 
including mine, are individual preferences and certainly are worth what 
you paid to receive them.

(1)  I' never upgrade an xp operating system to Vista.  If you want Vista, 
go with a machine that was manufactured with Vista in mind.

(2)  The time to migrate is when either your current machine is nearing 
the end of its life or you come upon some software, hardware, or task that 
makes your current machine obsolete.

(3)  I can rationalize buying a Vista machine (home premium minimum) to 
ease the learning curve and hardware/software transition giving you time 
to slowly move what you have on your existing machine to a new one.
I don't know about your machine, but I have a lot of things that are 
customized to my liking on my pc including how programs, hotkeys, 
utilities work, and all the rest.

Finally, in the blind community, every new operating system introduced 
brings up cries in just how difficulty it will be to use.
The facts that Vista, and xp before it, hardly were worth the cries of 
worry;
you may not yet want Vista, but blind usability shouldn't be one of your 
reasons.

If Vista gives you something you want or need today, great.  If not, wait 
and the machine you buy 2 months from now, 6 months from now, 1 year from 
now, etc will be all that much better.
