Fixed in trunk (r1933492) and 2.4 (r1933493).

The vhost common log format string in the Virtual Hosts section of the Logs
documentation was missing %h (remote host) and %p (server port). The format
string has been corrected from:

"%v %l %u %t \"%r\" %>s %b" comonvhost

to:

"%v %p %h %l %u %t \"%r\" %>s %b" commonvhost

Also fixed the typo in the nickname ("comonvhost" → "commonvhost").
