While addressing this, we also noticed that "per-directory context" was
used throughout the rewrite docs without ever being formally defined.

Added a glossary entry for "Per-directory Context" that explicitly
states it includes <Directory>, <DirectoryMatch>, <Files>,
<FilesMatch>, and .htaccess — and linked the key references across
the rewrite documentation to it.

This should help avoid future confusion about what "per-directory"
encompasses.
