Patched in 1933890 and 1933891

"per-directory" now always links to the Glossary, since it's more than just
htaccess files.

I could not figure out what the example was intending to illustrate, and what
the misconfiguration was. I have made a note to add a more useful example soon,
but for now the example has been removed.
