Cookies *are* headers, but I suppose I don't object to that too much. But that
wasn't intended to be a comprehensive list, so I'm not sure that we need to add
stuff. I suppose that could go either way.

But relegating pathinfo to only CGI is confusing, and nobody uses CGI any more,
so they'll think "that doesn't apply to me," when it in fact might. Augmenting
the mention of query string by saying query string again also doesn't appear to
accomplish anything.

The last part of the diff is just changing line wrapping, right?
