entirely duplicates the "Per-directory Rewrites" content from
rewrite/htaccess.html and also duplicates the "What is matched" section that is
covered in rewrite/intro.html

This content needs to be consolidated in one place (preferably in
rewrite/intro.html and rewrite/htaccess.html) and referenced from the other
place. The content in mod_rewrite.html is less clear, and so is what should
probably go away.
