mod_backhand is peer based. It is "up" if Apache is "up" as it is a part of Apache. Aside from that, it doesn't do any health checks. You don't go about configuring mod_backhand to balance requests across a cluster of servers. You install mod_backhand on each machine and they see each other and start balancing.
