I am wondering if in the loop:
 APR_BRIGADE_FOREACH(e, b) {

If for some reason the loop is being exited to early. 
This is my current thought, but for my environment I am not getting the
files cut off. What version of Apache are you using and have you
upgraded at  any point between version?
