4.0.1a not handling .htm files

Hi,
	I've just migrated a machine to apache 2.0.50 with mod_layout 4.0.1a and managed to get most of the configration from the apache 1.3.x setup working fine. However I've just discovered mod_layout is apparently ignoring .htm and .php files for layout directives, .html are unaffected. How do I force mod_layout to do any work on these files now that the directives:

LayoutHandler
LayoutDefaultHandlers
LayoutHTTPOverrideHandler
LayoutHTMLHandler

No longer seem to exist?
