I too have the same issue.  Basically the original page shows three
times (i.e. original plus 2).  I hadn't had much time to troubleshoot,
so I haven't posted it to the list.
I'm using "AddOutputFilterByType LAYOUT text/html" too, but specifying
a header and footer file.   I'm using the Apache bundled with RHEL 4,
which is 2.0.somthing-or-other (52?)
I had only noticed this problem on URLs that didn't directly specify
the filename (foo.com/bar/  as opposed to foo.com/bar/index.html), but
take this with a grain of salt since I really didn't have much time to
check other things out.

BTW - thanks Brian for fixing the tex/html problem back around
Christmas time...I didn't get a chance to tell you the fix you
supplied worked....the problem noted above did crop up, though.
