I find that since the changes made in
http://hg.tangent.org/mod_layout-5.0/rev/f7d637654371, if the content exceeds
8KB for PHP pages, the first 8KB is missing from the output and the
headers/footers do not appear.

This is with PHP compiled as an Apache module.  Similar results for PHP as a
CGI.  Plain HTML files to not exhibit this problem.
