mod_log_sql 1.100 Problems

I'm trying to get 1.100 to work correctly.  The modules load and apache 
starts, however, nothing ever hits the database and everything goes to 
the preservation file.

I've tried the precompiled debian packages as well as compiling the 
modules myself - results are always the same.

I can connect as the mysql user to the database server using the 
password in the apache conf file.  No problems there.

mod_lod_sql doesn't seem to provide much debugging information, so, I 
don't know where to turn to start looking for issues.

Help?
