I know this to be on the list of to-do items, but don't hold your
breath.
Conceptually, this is really simple to do, Apache could just use a FIFO
pipe internally to serve as a mp3 fountain of bits for the Apache
children processes to feed from. Then, when user agents invoke Apache
child process, they just drink from the fountain. On the flip side, a
way needs to be thought of to supply the fountain of its bits. So far
the goal is to support the shout program style of doing things. That way
we wouldn't need to support any sort of proprietary software to do this,
and instantly support a wide order of preexisting streaming tools in the
wild already.

Is it possible to have mod_mp3 send out a synchronized stream of music?
ala "shoutcast".  Or is that what the unused MP3Stream directive is for?
