Yes, it should look the same. Anyway, if it didn't you should get a 
document error, not a crash.

I'm sorry, but I don't know what is wrong. Not unless you have an old 
survey.conf somewhere (for example in /etc/apache/conf/conf.d) or have 
upgraded apache strangely somehow (ie having disabled basic auth or 
something).

We'd have to debug it in a more structured way, which is difficult to do 
remote. What I can suggest is trying to re-install the old tarball, and if 
it works again, try to figure out what the difference is. If it doesn't 
work, then I'd try to figure if something has changed with apache or 
apache's config files.
