Re: integration with contacts

I don't understand what it is you want to do. Do you want specific 
questions for contacts? Or authentication so you know who answered the 
questionnaire?

If you want to import data from a database into a survey, you can use the 
IMPORT tag. This is useful to, for example, populate a LIST with names of 
people or similar.
