These are two different problems. I've noticed the my things earlier but 
since it didn't do any actual harm I ignored them (they're just 
superfluous declarations of local variables).

Concerning the other problem, my first guess would be that the 
PerlSendHeader flag has the wrong setting in survey.conf. Try switching it 
and restart apache. Although this usually do not show up as a download 
dialog, so I'm not sure it is the same problem.

If it doesn't work, please send the output of:

   lynx -head http://127.0.0.1/mod_survey/examples/example.survey

(run on the server)
