There's also an attachement with a sample survey.
The included script for calculating the progressinformation is very 
outdated and will run out of memory on surveys with lots of branching 
(not to mention the time it takes)
You could also have a look at ILSE (http://ilse.eveca.de) where this is 
fixed. With ILSE it is also possible to set SURVEY parameters an 
SECURITY settings globally in all files of a survey chain (for example 
the ILLEGALVAL from above).
