Keith,This looks like a Windows driver and/or startup sequence issue.  You got it to work after reinstalling the driver that came with the mixer.Check the driver again.  Reinstall if necessary. Perform a clean reboot (no USB attached).  The driver should not show in Device Manager.  Plug in the USB, check the driver.  If the wrong one is loaded, you may need to update the driver.
