Wow. That's a collection of several different problems.
Growing since years.

Very funny are the many "the same error" statements.
(All problems are the same. But some are more same than others.)

"SK=5h/ASC=64h/ACQ=00h" means that the drive does not like the
state of the medium: "ILLEGAL MODE FOR THIS TRACK".
One can get this when attempting to write to non-blank DVD-RW.
It can even be caused by automounter interference.

":-( unable to WRITE@LBA=0h: Input/output error"
Could be caused by an automounter, too. If it was a DVD-R[W] medium.

"COMMAND SEQUENCE ERROR" @LBA 0h. Is media being read?"
And again an automounter.
(They are so shitty that they change with every distro release.)

":-[ WRITE@LBA=200h failed with SK=4h/ASC=08h/ACQ=03h]: Input/output error"
Is a hardware problem: "LOGICAL UNIT COMMUNICATION CRC ERROR (ULTRA-DMA/32)"

":-[ WRITE@LBA=0h failed with SK=3h/ASC=73h/ACQ=03h]: Input/output error"
The medium is worn off "POWER CALIBRATION AREA ERROR".

"mkisofs: Directories too deep"
Is not related to burning but to ISO 9660 production.

"Sense Key: 0x3 Medium Error, Segment 0"
A rotten medium reported by wodim.

"Sense Key: 0x5 Illegal Request, Segment 0
Sense Code: 0x27 Qual 0x00 (write protected) Fru 0x0"
I can only guess what the drive might mean with "write protected".

"BraseroGrowisofs stderr: :-[ WRITE@LBA=0h failed with SK=3h/WRITE ERROR]:"
Drive hates medium.

"Sense Key: 0x5 Illegal Request, Segment 0
Sense Code: 0x21 Qual 0x02 (logical block address out of range)"
Probably an automounter interfering.

"md5sum: /dev/scd0: Input/output error"
One should eject and reload before accessing freshly written media
via the block device driver.

":-[ WRITE@LBA=1d3500h failed with SK=3h/POWER CALIBRATION AREA ERROR]:"
Rotten DVD-R DL medium. (Died out, commercially.)

":-[ WRITE@LBA=10h failed with SK=0h/ASC=00h/ACQ=02h]: Input/output error"
This one could be ours. But it comes too early.
Note the implausible SK=0, which means "no error". I suspect "Descriptor".

":-[ WRITE@LBA=1f0e80h failed with SK=0h/ASC=00h/ACQ=02h]: Input/output error"
Once again the ill SK=0. This time at the layer break of a DVD+R DL.
No DVD DAO. Surely aligned to 16 blocks.

Have a nice day :)

Thomas
