Programming resources

Wondering if I could get any recommendations on resources, print or online,
for learning more about programming midi and sound in C and C++ for someone
who is, let's say an intermediate C programmer, beginner C++ programmer. I'm
hoping to find something that will help me make the jump from what's in
regular programming books to reading ( and hopefully writing )  source code
for things like Csound, Blue, Ardour, PD, etc.

I've read some great books on advanced C fundamentals, but don't know how to
apply that stuff to timing critical multi-threaded apps, or how to work with
audio and midi ports, pipes, sockets, blah blah!
