i put up a commandline version (4.23f07) that includes CoreMIDI support
here:

http://csounds.com/matt/csound.sit

i had the best luck by putting it in the  /usr/bin directory - i dont know
if thats the proper place or not.  you may have to change the owner and/or
permissions too..
