A comment to question 1b, in comparison to 1d: 
Let's start from the end. 
1d - "copy OK" means your copy is identical to original, as far as EAC algorithm can say. 
1b - "Track quality" is lower than 100% when EAC have to reread sectors in order to have equal results and consider the copy OK. 
So, 90% quality does not mean your copy is different from original, but, instead, it means EAC encountered problems in reading track and had to retry until a certain number of results have become reliable. 
In fact, your copy is BETTER than original, because it can be probably extracted by EAC with a 100% quality....

a. Peak level is how loud the loudest portion of the rip is in terms of 
the maximum possible. 

b. Not positive on this one, but I assume it's the number of sectors 
that were read without any error. 

c. The confidence level on AccuRip reports is the number of other rips 
submitted to the database that are identical to your rip. 

d. I assume "Copy OK" means that EAC was able to read all sectors 
without error (eventually, in the case of sectors that had to be re-read). 

2. EAC is telling you that all extracted tracks were reported as 
bit-for-bit accurate by AccuRip database. 

3. It means the same thing as the confidence for the AccuRip database, 
but in reference to the Cuetools database.
