Let me know if you solve this problem, but I think it's nearly 
impossible to solve. I have some discs ripped decades ago to individual 
tracks that I'd like to "convert" into an accurate single disc image. 
The problem is that ripping to individual tracks throws away any 
pre-track gaps. They are lost forever. If all of the pre-track gaps on a 
disc are just pure silence it may be possible to reconstruct the disc 
image, but so far I have been unsuccessful in doing so. I've even tried 
manually finding the exact pressing that my tracks were ripped from and 
using the TOC data to reconstruct pre-track gaps.
