    *  Moved global bypass button next to limiter bypass and made it
       a check box

    *  Global bypass blinks every 1/2 second when on

    *  Added main output level numeric boxes above output meter

    *  Added RMS level numeric boxes above output meter

    *  Added ability to reset single RMS or output peaks in output meter
       by clicking in the numeric level boxes

    *  Added ability to select output/RMS level or output/RMS peak
       levels in the numeric level boxes from the preferences dialog


    Still need to allow setting of the RMS window size by number of
samples with an associated display of the time window this will create.
Right now the RMS meters are hard-wired to 1000 samples.  I'm thinking
that we should limit the RMS window range to between 1000 and 5000
samples.  This gives us a time range of between 0.0227 and 0.1134
seconds at 44100Hz and between 0.0104 and 0.0521 seconds at 96000Hz.
Does this sound OK?
