If you're trying to burn an audio CD, try:

     madplay -o cdda:track.cdr

The result is a raw CD audio track ready to be burned.

If you really must have a WAV file, you'll have to specify all the 
parameters yourself:

     madplay -o foo.wav --stereo --bit-depth=16 --sample-rate=44100
