Which version of LAME is recommented nowadays ?

I've been using 3.93 for a long time. 
Is there any serious errors in terms of sound quality in this version
that I should be aware of ?

Another question

How should the VBR encoding output from LAME be interpreted. I've looked
around but I didn't find anything on that.
What does the ###* mean?
