This is a problem with iTunes' handling of .m3u playlist files.  I had 
a friend at Apple look it up and this bug had not yet been logged, so 
we created an extremely detailed bug report for it.  Hopefully this 
should be fixed by Apple soon.

In the meantime, I've set my OS X machines to use RealOne for .m3u 
playback -- not my favorite player, but it works for now.  :-)
