A proper atomic mkstemp shouldn't be too hard.  tmpnam, open is
probably acceptable, since we're not using it for anything secure, but
I'll treat that as a fallback solution.  I'll likely make a new
"posix" library to hold mkstemp and anything else we find is missing
from Win32 (although I don't intend for it to turn into Cygwin!)
