documentation changes

I moved some of the documentation files out of the top level
directory and into the 'doc' subdirectory.

The make procedure will attempt to make programmer level
documentation, which is probably not correct, but I am
waiting to see a better way.

A question for the automake gurus:

   I've made a stab at using automake to generate
the docs using doxygen.. I don't think it's right but
it's there.  Fixes are welcome.

   Doxygen creates a html subdirectory in the directory
that it is run.  How do I tell automake to get rid
of this directory on 'make clean'
