Since no one else dared to answer your problem, I will try. :-)

First - what system do you have? Kind of Linux?
You are using KMail, so I suppose it is some Unix-like system.
In that case I am almost sure that you should have some flavour of sendmail 
installed. Maybe with another name?

As to the question how gnats sends out mails: as far as I know it calls that 
'sendmail' program and lets it dispatch the message.

I am not sure whether gnats is able to send mail in some other way, but as 
soon as I get back home, I will dig into the problem...
