What about top_include_page? It doesn't seem to make a difference
as there's no icon on top anyway. Even without include_page or
logo_image set the mantis logo doesn't show up (on top). I also
tried logo_image but that wasn't shown either.

I know, we've been using them for years with the old installation.
The option is also enabled. I'll try to find out what happens next
