in-reply-to handling

I saw that in the recent development version, in-reply-to handling (for
matching replies to previous emails when there is no designator in the
subject) has been changed (or was it newly implemented?).

Since I wasn't aware this feature existed, some questions on this:
- Does this take into account that a tracker may have multiple
  issue-like classes and the message might be attached to one of them?
- Do we have an index on msg.messageid? I have a tracker with around
  2 million messages. So if this does a full table scan on each incoming
  message that wouldn't be too nice :-)
