how to use path-forms to join a mailing-list or to feed dbs?

hi

i just want to get two kinds of forms on my bxcms-site:

# first:
a small form to let users easily subscribe (and of course  
unsubscribe) to a mailing list (using mailman e.g.)

# second:
a "medium-size" form to feed a mysql db (and maybe on the same time  
to mail it, too - depends on the situations of use...)

how can i get solved this problem?
