We did the human cut-and-paste method. Everyone had to be set up in the
new CMS and then they had to manually migrate their content from the old
site to the new. Using Xstandard as our inline editor, we were able to
clean up most if not all of the old cruddy HTML.

Of course, this only works if your authors only need to migrate dozens
of pages (rather than hudreds or thousands). The one benefit for us,
though, is that it forced everyone to finally look at their web content
again and make the appropriate updates that they had been neglecting to
do for some time.
