we are currently implementing Apache Lenya 
(http://cocoon.apache.org/lenya/) for the Children's hospital of Zurich 
(Switzerland)

http://www.kispi.unizh.ch (still the old site)

whereas Lenya is Java, XML and XSLT based, but by using Apache with 
mod_proxy
you can also bypass PHP and other programming languages, or if one is
generating well-formed XML with PHP for instance, then you can proxy it 
through Cocoon/Lenya which allows smooth integration of various 
technologies resp. possible migration within the future.
