Actually, access keys should probably be avoided since they can 
interfere with the keyboard shortcuts used by screenreaders and the
like.

If a site is designed effectively and with the end user in mind (abled
or not), I see no reason why any accessibility features cannot be
implemented into a single version of a document. Some features such as
fly-out menus that people want to throw into their designs actually
prove to be less usable even for perfectly abled users.
