Thanks Justin and Geoff.  I will hope off on logging tickets.
 
With the image, yes.  I've attached a screenshot where I had the cursor on 
the 4th line and then selected insert image.  In the second screenshot you 
can see it's inserted it in the wrong spot.
 
Glad to hear the create/select button will change as it wasn't always 
contributors that had to stop and think, I would often do it myself. :)
