Build a jahia form which can be used as a suggestion box

Hi all, 

 

We want to build a form in our Jahia site (for one of the main tabs)
which can be used as a suggestion box.

The form would have the following fields:

 

Name

Site Name

Comments

Submit

 

Can anyone please suggest me how to go about these changes? What will be
the template and database schema changes that need to be made for the
same? What is the best way of integrating it with the existing jahia
site.
