Most developers are not good at planning, lets admit it
But maybe thats just my experience, apologies for the generalisation

But then the  clever author just seems to think of another way to achieve

I think we are getting to the crux. A system needs to be specified by its 
users
Yes, authors are not the only users. And programmers generally are not users 
at all.
Definitions like CMS and DMS do not identify sufficiently users tasks
There are different classes of users,   they need to be identified and 
queried at planning stage.
Only when they are all satisfied, the system fulfils its purpose
