The changes in the Tiki files are fairly straightforward, I think. There 
are a lot of CSS class changes in templates that will be just 
find-and-replace, but also some HTML changes involving forms, navs, and 
so on. But it's manageable. In the experimental branch, I've been 
updating the themes files (less to scss), and making a few changes 
outside of the themes directory as described on the themes.t.o page.

For sites that use Bootstrap 3 grid classes in wikitext, there shouldn't 
be trouble after the update. I did a test by copying the grid columns on 
https://getbootstrap.com/docs/3.3/examples/jumbotron/ to a wiki page in 
my local Bootstrap 4 experimental branch, and the columns are still 
arranged correctly because the old grid class names are still valid 
(although the layout method has changed to flexbox). Bootstrap 4 also 
adds new classes for more layout options, based on flexbox.

If site editors/authors hand-input certain old BS3 HTML, like for 
dropdowns or tabs, or use "panel" classes (replaced by "card") these 
will break and need to be updated; also the "well" class is no longer 
supported, so won't get the expected design details. But of course the 
wiki plugins to create this HTML will be getting updated (some already 
is) so no problem for the site users.
