You need to :
a) upgrade the comments module after doing your site upgrade (and upgrade
   any other module that says it has an upgrade)
b) check your comments templates (if you override them in your theme) to make
   sure that you have the hidden field header[itemtype] in the forms.

There's a message about that in the xaraya.users newsgroup from 20/10/2003
"Template changes in comments module for .9.1.3" - I'll leave the translation 
to marc :)
