sent additional information to sitecontact form

hi everybody,

I'm trying to do some feedback for my site and I want to put a link to
a sitecontact form on some pages, so when a tester click on the link
to the sitecontact form they will just need to add their feedback and
some data (what module was he viewing when clicked to the form, etc.)

Any idea how could I sent that extra data?
