Here are the exact steps:
1. Turn off variable substitution in Toad
2. Checkout a package body that has & in front of a literal string that is in lowercase (for example &country)
3. Make a change and save it to the database
4. Undo the checkout

After I do this the literal string with the & in front of it has been changed to uppercase
