I will e-mail you the patch tomorrow morning.

There is one possible problem with my implementation of NTLMv2. I do not 
  include the LM-part in the response. Doing this has been preventing 
authentication in case of combination of our domain controller and SQL 
Server. Theoretically, LM-hash is supposed to be included in the response.
