You might need to compile the MSSQL extension with the location of FreeTDS passed to the ./configure script. You may also have to set Apache environment vars to know where the FreeTDS conf file is. I had to do that when getting it set up on Ubuntu. Not sure if it will be exactly the same for mac, but these steps are the ones I followed to get PHP talking to MSSQL on my Ubuntu machines.
