Yes, this it seems to not be picking up my config file properly.  My
freetds.conf file is located at /etc/freetds/freetds.conf, but
apparently, freetds looks to /usr/local/etc/freetds.conf when
$FREETDSCONF is not set.

I tried setting FREETDSCONF, and I tried putting my server definition
in  /usr/local/etc/freetds.conf, and both solved my problem. (I tried
them separately, so either one solves my problem.)
