mssql does show up using the php -m (there was an intervening /bin
directory in there somewhere)

and otool shows:

/Applications/MAMP/bin/php5/lib/php/extensions/no-debug-non-
zts-20060613/mssql.so:
	/Applications/MAMP/Library/lib/libsybdb.5.dylib (compatibility
version 6.0.0, current version 6.0.0)
	/usr/lib/libiconv.2.dylib (compatibility version 7.0.0, current
version 7.0.0)
	/usr/lib/libgcc_s.1.dylib (compatibility version 1.0.0, current
version 1.0.0)
	/usr/lib/libSystem.B.dylib (compatibility version 1.0.0, current
version 111.1.5)

and I agree that MAMP has complicated the system, but as an end user,
I didn't notice it because it's all hidden until I try to do something
like this :P Why can't software all just get along?

and thanks for your ongoing help with my confused system :)
