I've traditionally run the svscanboot script from inittab, which does
the above and runs readproctitle as well.  This is typically set up
by package/install, but if you're using a distribution package then
that step has possibly been overlooked.

BTW, I don't think you want to be running svscan in run level 6, or
probably 1 either.
